Understanding Cost and Coverage

Our Chronic Care Management (CCM) service is covered by Medicare and applies to your annual deductible and coinsurance (typically 20%). If you have a secondary or supplemental insurance plan, it may cover your coinsurance, reducing your out-of-pocket expenses. While you can only enroll in CCM services with one of our CareHarmony Care Coordinators, participation is entirely voluntary. You can opt out at any time—no obligations or hidden fees.

Who is eligible?

Medicare beneficiaries with two or more chronic conditions expected to last at least 12 months or until end of life qualify for the Chronic Care Management program. Contact your primary care provider to confirm your eligibility.

Do I have to be a Medicare member?
