Our Chronic Care Management (CCM) service is covered by Medicare and applies to your annual deductible and coinsurance (typically 20%). If you have a secondary or supplemental insurance plan, it may cover your coinsurance, reducing your out-of-pocket expenses. What is care coordination?

Medicare's Chronic Care Management program offers this service, providing extra support between doctor visits for patients with two or more chronic conditions.

Who is eligible?

Medicare beneficiaries with two or more chronic conditions expected to last at least 12 months or until end of life qualify for the Chronic Care Management program. Contact your primary care provider to confirm your eligibility.

Do I have to be a Medicare member?

Yes, this program is currently only available to Medicare patients.
