If you have had a positive COVID test for these symptoms more than 10 days ago, you must bring those results with you to the visit.
If you have NOT been tested for COVID and your symptoms are less than 2 weeks old, you must get a COVID test (rapid) from your PCP or Urgent Care within 48 hours of your appointment. You must bring your NEGATIVE COVID results with you to the appointment.
If this COVID test is positive, you must reschedule your appointment for 10 days after the positive test. Seek treatment for COVID from your primary care physician.
If we are unable to accommodate the above request, our scheduler will assign you to the next available appointment with the provider you have requested.
Texas law requires the Texas Department of State Health Services to collect information on the race/ethnic backgrounds of patients. Medical facilities are required to ask patients to identify their own race and ethnic backgrounds.
The data obtained through this process will be used to assist researchers in determining whether or not all citizens of Texas are receiving access to adequate health care.
